Download Pathway Agreement

Downloading Instructions

We have placed our Agreements and Forms here on the website for you in PDF file format*. View or download any of them using one of the following methods, or simply contact us to have them sent to you in the post.

Your options are:
  • View an item on-screen by clicking the link to open and view the file, or;

  • Download an item to your computer by right-clicking the link.
    Select "Save target as..." and choose a location on your computer to save the file, or;

  • Download an item to your computer as a compressed zip file by clicking the 'zip' link [This option will require winzip decompression software on your computer]

  • Download times will vary depending on your internet connection speed. As a guide, 1.0 mb file usually takes 3 minutes on a 56.6k modem.
*All options above require the free software 'Adobe Acrobat Reader' to be installed on your computer. Your computer may already have it. If not, click here or the image to download Adobe Acrobat Reader.
